Instructions for Official Form 121: Statement About Your Social Security Numbers

Attorney Filer:

  1. DO NOT electronically file Official Form 121: Statement About Your Social Security Numbers
  2. Retain a signed copy of the Statement About Your Social Security Numbers (Official Form 121), with full SSN notated, at the Debtor’s attorney’s law office for a period of five (5) years. See General Order 24.
  3. Retain a signed copy of the Debtor’s Statement of SSN (B21), with full SSN notated, at the Debtor’s attorney’s law office for a period of five (5) years. See General Order 24.

Amendment of a social security number - Attorney Filer:

  1. Docket the Request for SSN Modification event under Bankruptcy>Pleadings>Miscellaneous>Request for SSN Modification.
  2. Email the Amended Official Form 121 to dropbox@areb.uscourts.gov.
